Enrollment procedures for 10th through 12th Grade Students

Procedures for enrollment for 10th through 12th grade students new to the San Mateo Union High School District.

Upon providing proof of residency (explained below) and completing the enrollment process, you will receive a Permit to Register which is also required should you wish to participate in the Districts Open Enrollment (Intra-District Transfer) Program.  The Intra-District policy, within certain guidelines, allows a family to request enrollment for their student at a District high school other than their assigned school of attendance.  The Permit to Register would need to accompany the transfer request. However, as stated above, in order to participate in the transfer process , you must first officially enroll in the District and demonstrate proof of residence. The Permit to Register would need to accompany the transfer request.

Materials Required to Demonstrate Proof of Residence – Bring Copies and Originals

If you own your home, you MUST provide:

  • a Federal or State Tax Return -  NO P.O. BOX ADDRESSES WILL BE ACCEPTED
  • AND

    • a current Deed of Trust or Property Tax Bill or a Final Escrow Letter or a Tax Assessment Card as your proof
    • a Picture ID is required or Drivers License or Passport

    If you rent, you MUST provide:

    • a Federal or State Tax Return -  NO P.O. BOX ADDRESSES WILL BE ACCEPTED

      AND
    • a Lease Agreement – should include name of the homeowner or property management company and their phone number
    • a picture ID is required or Driver’s License or Passport

    In the case where no Property Tax Assessment, Escrow Letter, Deed of Trust, or Lease is available:

    • you may use a current W-2 Form with your current address as your proof of residency
    • a picture ID is required or Drivers License or Passport

    And

    Required Student Records

    • Withdrawl from previous high school
    • Current transcipt
    • Current Immunizations
    • Birth Certificate or Pssport
    • Please be aware that you may be asked to provide further documentation.

    IF YOUR STUDENT IS COMING FROM A PUBLIC SCHOOL NOT LOCATED IN THE SAN MATEO HIGH SCHOOL DISTRICT OR IS A STUDENT COMING FROM A PRIVATE SCHOOL, PLEASE BRING THE ABOVE PROOFS OF RESIDENCY AND THE FOLLOWING TO ANY ENROLLMENT CENTER IN ADDITION TO THE ABOVE REQUIRED DOCUMENTS:

    • Birth Certificate or Passport;
    • Current Immunizations
    • Latest Progress Report or Report Card if entering 9th grade
    • Transcript (if high school student)

    PROOF OF RESIDENCE WILL BE HANDLED AT THE DISTRICT OFFICE AFTER MARCH 9, 2007, located at 640 N. Delaware Street, San Mateo. Hours after March 9, 2007, will be 8 a.m. to 3 p.m., Monday through Friday.
